A long-term condition (LTC) is a health problem that has a big impact on your life and is not going to go away.

LTCs include conditions such as emphysema, asthma, arthritis, diabetes, chronic pain, mental health conditions, heart conditions and many more. As you get older you may have more than one LTC.

LTC nurses support people living with long-term conditions to gain and grow the knowledge, skills and confidence they need to manage their symptoms.

LTC nurses may have different titles and a range of skills, with some focusing on certain conditions. Some nurses you may see are:
  • Nurse practitioner
  • Clinical nurse specialist
  • Clinical community nurse specialising in LTCs
  • Disease state management (DSM) nurse
  • Practice nurse
The person you will see will depend on the type and number of conditions you have, and the general practice you attend.

How can the LTC nurses support me?

You may be referred to a LTC nurse if you need more intensive help, support, or education to live well with your conditions. The focus will be on helping you to look after yourself better.


Usually LTC nurses will start with an assessment to learn about what you are coping with, how you are managing your conditions, and how they affect your life.


Together you will develop a plan for how to address some of your challenges. The nurse will provide the education and support you need over this time, and your general practice team will be included in any big decisions that are made.

How can I access support from a LTC nurse?

Referral from your General Practice team


You will be referred to the service by your General Practice team or health provider. Talk to them about the service if you think it is something that would benefit you.

When and where?

This will depend on which nursing service you have been referred to. Most people will be seen at their general practice, but some may be seen at THINK Hauora, or at home.

Does it cost anything?

If you see a nurse employed by Te Whatu Ora MidCentral, THINK Hauora or through a Māori or Iwi service there will be no charge. Some fees may apply if the nurse is provided by a general practice.
