The Map of Medicine (MoM) tool was disestablished in June 2018. It has been replaced by Whanganui & MidCentral Community HealthPathways, with new localised pathways in development at all times.

 All pathways that are now live on Whanganui and MidCentral HealthPathways have been removed from this page.

Below are localised MoM pathways developed before 2018 that have not been reviewed or updated since. Therefore, users of MoM pathways are urged to employ clinical judgement to ensure current best practice.
