Horowhenua Community Practice (HCP) is owned by THINK Hauora (PHO). The day to day operations of this General Practice is led by a collaborative Management Team consisting of a Practice Manager, Clinical Nurse Lead and Team Leader Administration.
HCP is the largest general practice currently operating within the Horowhenua District and is also the site for the centralized Afterhours Clinic for weekends and public holidays. HCP has three general practitioners and two nurse practitioners supported by a team of experienced nurses and admin/reception staff. HCP offers a wide range of services including those focusing on long term conditions.
To provide competent patient-centred care that addresses health needs and promotes wellness. Such care is based on latest evidence and guidelines and meets legislator requirements.
