Earlier this week, Ashleigh Blackbourn, Clinical Lead - Acute & Health Care Home and Ehsan Tehseen, Network Sustainability Manager presented Cook Street Medical with their Health Care Home Credentialing.

There are three levels of achievement: Credentialing, Certification, and Re-Certification. 

Cook Street Health Centre have worked incredibility hard over the last year at improving the service they provide to their patients and whānau. This has included:

• improved acute demand access and management
• patient and whanau engagement to develop equity-based initiatives and co-design
• providing extended hours for routine care
• utilising telehealth to provide alternatives to face-to-face appointments
• promoting portal usage to remove barriers and empower patients to manage their health information
• regular review of call metrics to ensure patients have timely access to health services
• implementing Lean methodologies such as visual boards, rooms standardisation and daily huddles to improve workflows
