Cook Street Health Centre Achieves Credentialing

Congratulations to Cook Street Health Centre who have achieved the Credentialing level of our Health Care Home (HCH) Programme.
Earlier this week, Ashleigh Blackbourn, Clinical Lead - Acute & Health Care Home and Ehsan Tehseen, Network Sustainability Manager presented Cook Street Medical with their Health Care Home Credentialing.
There are three levels of achievement: Credentialing, Certification, and Re-Certification.
Cook Street Health Centre have worked incredibility hard over the last year at improving the service they provide to their patients and whānau. This has included:
- improved acute demand access and management
- patient and whanau engagement to develop equity-based initiatives and co-design
- providing extended hours for routine care
- utilising telehealth to provide alternatives to face-to-face appointments
- promoting portal usage to remove barriers and empower patients to manage their health information
- regular review of call metrics to ensure patients have timely access to health services
- implementing Lean methodologies such as visual boards, rooms standardisation and daily huddles to improve workflows
